Why PeriNext exists
Periprosthetic femoral fractures — fracture occurring around an existing hip implant — are one of orthopedic surgery's most demanding challenges. They strike a population that can least afford a long, high-risk operation: elderly, frail, often osteoporotic patients. Yet the standard answer has long been major revision surgery — removing a working implant, reaming the bone canal, and inserting a new, longer stem. Alternatively, a long plate is inserted to lock the bone into anatomical plane.
PeriNext set out to change that equation. Our patented periprosthetic nailing system stabilizes the fracture by engaging directly with the implant that is already there — through a closed, minimally invasive technique that leaves the existing stem, the surrounding bone and the patient's recovery window intact. We have completed four first-in-human cases in Israel.
The technology
PeriNail is inserted through a closed intramedullary technique and merges coaxially with the femoral stem already in place — turning two separate components into one stable construct, without ever taking the original implant out.

No stem removal, no canal reaming, no periosteal stripping. The existing implant stays in place and the patient keeps their bone stock intact.
